So the solution is simply pick up a death machine power up. However, I have only seen one comment on this other solution to the bug where I also figured it out in game before seeing the comment but, the comment did make me realize that's what was fixing it for me. I looked online and found that the most common answer to this was to just down yourself which would reset the equipment held in your hand.
I was doing the Revelations EE today for the first time and found out that there is a bug with the summoning key when you try to teleport to Nacht der Untoten with it making it so that the game thinks you have it even though you are not holding it.
